Upper School Checklist

RAVENNA ACCOUNT
Applicants must have a Ravenna account to access our online admissions portal. Once your account is active, you’ll see more details and may register for events.
PRELIMINARY APPLICATION
Our application opens on September 3, 2024 and includes some background information.
Grade 9 Deadline: November 22, 2024
Non-entry Years (Grades 10-12) Deadline: November 8, 2024
IN-PERSON STUDENT INTERVIEW
For applicants to Grade 9, families (student and at least one parent/guardian) are required to register for a one-on-one in-person interview with a member of the admissions committee. For applicants to Grades 10, 11 and 12, interviews are based on space availability and are by invitation. The interview will include a short writing assessment. Interview dates will be available starting on September 3, 2024.
STUDENT WRITING SAMPLE
Please upload a sample of written work completed by your child as a school assignment during the current academic year. The sample may be an essay, paragraph or response/reflection. We understand that there are a wide variety of ways that written work is assigned and teachers provide feedback. We prefer that the writing sample include the description of the assignment and the teacher comments/feedback/grade if available.

PARENT/GUARDIAN SHORT ANSWER QUESTIONS
Families are asked to complete the ISAAGNY Parent Statement Form.

FALL REPORT CARD
Upload your child’s fall progress report from the first marking period. Please contact our office if your child will not receive a report card by this date.

GENERAL STANDARDIZED TESTING SCORES (Grades 10-12)
Submit standardized test scores from the PSAT, SAT / ACT, or Regents.
ISAAGNY GUIDANCE COUNSELOR RECOMMENDATION FORM (Grades 10-12)
Please delegate this form to your student’s dean, guidance counselor, advisor, or division head.
FINANCIAL AID APPLICATION
Families who would like to apply for financial aid must submit an application in Clarity by December 15, 2024.
ISAAGNY SCHOOL COMMON ESSAY
Students will be required to respond so several chosen prompts to complete short essays that will be shared with other schools utilizing the same form.

TEACHER RECOMMENDATIONS
Poly Prep requires Math and English teacher recommendations. If both subjects are taught by one person, that teacher may complete both recommendation forms. We strongly recommend that you request the recommendations after October 15 to give the teacher enough time to get to know your child and complete the form before it is due.

TRANSCRIPTS
Download the Transcript Request Form in Ravenna and print or email it to the records office at your child’s current school after December 1.

ISEE or SSAT
Testing is required for admission into all Upper School grades this year. You can read more about our new policy here. Poly will superscore multiple test score submissions. Families are welcome to submit scores for either the SSAT or ISEE.
